Suicidal Ideation Questionnaire (SIQ)
William M. Reynolds's Suicidal Ideation Questionnaire (SIQ) helps professionals assess how often adolescents aged 12 to 18 have suicidal thoughts, so that troubled young people can be evaluated and monitored. Because depression and suicidal thinking do not always go together, it adds something that a depression measure alone cannot. The SIQ is for grades 10 to 12 and the SIQ-JR for grades 7 to 9; items are rated on a 7-point scale, it takes 10 minutes or less, and the kit contains the professional manual, 25 hand-scorable answer sheets and a scoring key for each version.
