What triggers a referral
A screening result crosses the threshold set for that instrument.
Something disclosed in a session falls outside what career guidance can address.
The person asks for help with something the professional is not qualified to treat.
Risk of harm to the person or to another is indicated.
What happens then
Said in the room
The person is told, at the time, that a referral is being suggested and why. Not afterwards, and not in a letter they read alone.
Consent asked
Except where risk of harm makes it unsafe to wait — the circumstances in which that exception applies are for counsel to define.
Route named
A named professional or service, not a category. Handing someone the word “psychiatrist” is not a referral.
Warm handover where possible
An introduction, an appointment, or at minimum a name and a number that has been checked as current.
Recorded
What was said, what was agreed, and what was done.
Followed up
A check that the person reached the service, after a stated interval.
