Eating Disorder Inventory™–3 Referral Form Kit (EDI™-3 RF)
The EDI-3 Referral Form helps allied health professionals spot people aged 13 to 53 who already have an eating disorder or may be developing one. The short self-report form combines 25 EDI-3 questions with questions on eating-related behaviour, and the need for referral is judged from body mass index alone, from body mass index together with the answers on eating concerns, and from the behavioural answers. It is hand-scored, takes 5 to 10 minutes, and the kit contains the referral form manual and 25 forms.
