The Attention Deficit Disorder Evaluation Scale–Fifth Edition (ADDES-5) helps educators, psychologists, paediatricians and other professionals evaluate ADHD in children and young people aged 4 to 18, using ratings from the adults who see the child's behaviour every day. A School Version is completed by teachers and a Home Version by parents, and both score the Inattentive and Hyperactive-Impulsive subscales in line with DSM-5 practice. It takes about 30 minutes, and the complete kit includes the technical manuals and rating forms for both versions along with intervention documentation forms.
