Adolescent Symptom Inventory-4R

The Adolescent Symptom Inventory-4R (ASI-4R) by Kenneth D. Gadow and Joyce Sprafkin is a rating scale that checks young people for signs of the emotional and behavioural disorders defined in DSM-IV and DSM-5, for ages 12 to 18 at middle or secondary school. Parents complete a 120-item checklist and teachers a 79-item one, and the parent form also asks how much the symptoms of each disorder get in the way of the young person's social and school life. A clinician can read the parent checklist aloud, turning it into a structured interview. The kit contains a manual, 25 parent and 25 teacher checklists, and 50 each of the cutoff score sheets and the severity profile score sheets.
