ADHD Symptom Checklist-4 (ADHD-SC4)
The ADHD Symptom Checklist-4 (ADHD-SC4) tracks how children and adolescents aged 3 to 18 with ADHD respond to treatment, through 50 items that parents and teachers answer in the same version. Its four subscales cover DSM-5 symptoms of ADHD and of oppositional defiant disorder, a Peer Conflict Scale for aggression towards peers, and a checklist of stimulant side effects for monitoring medication. Scores can be read as symptom counts against cut-offs or as T-score severity ratings, and it takes about 5 minutes. The kit includes the manual and 50 each of the checklists, summary score records and severity profile sheets.
